Healthy Louisiana: The State's Medicaid Expansion Program

Louisiana expanded Medicaid under the Affordable Care Act effective June 1, 2016, under Governor John Bel Edwards — making Louisiana the first state in the Gulf South to do so. The expansion program is called Healthy Louisiana and covers adults between ages 19 and 64 with household incomes up to 138% of the federal poverty level. In 2024, approximately 2 million people — roughly 43.5% of Louisiana's population — were covered by Medicaid, making it one of the highest Medicaid coverage rates in the nation.

Louisiana's Medicaid expansion is administered by the Louisiana Department of Health (LDH) through managed care organizations (MCOs). As of the end of 2025, five MCOs serve Healthy Louisiana enrollees after UnitedHealthcare exited the program effective December 31, 2025. Medicaid enrollment is open year-round with no enrollment deadline. Residents apply through HealthCare.gov, which automatically checks Healthy Louisiana eligibility when income information is entered, or directly through the LDH.
